Hargraves Psychology is operated by Hargraves Psychology Professional Corporation. Heather Hargraves, M.A., C.Psych. is a psychologist registered with the College of Psychologists and Behaviour Analysts of Ontario (CPBAO) and is a health information custodian under Ontario’s Personal Health Information Protection Act, 2004 (PHIPA).

1.What this website collects

This site uses cookies that are necessary for it to function, together with the website analytics built into our GoDaddy plan, which counts visits and pages viewed. We do not use advertising or tracking cookies, and we do not sell, rent or trade anything collected here.

This site does not use a contact form. It displays our email address. If you write to us, we receive your email address and whatever you choose to tell us, and we use it only to respond to you. Please keep in mind that email is not secure, so do not send detailed personal or health information that way.

The booking link takes you to Jane, the scheduling and records platform used by the practice. Anything you enter there is collected by Jane under its own terms. See jane.app/privacy.

2.Your information as a client

Once you begin services with the practice, your information is handled under PHIPA.

We collect contact and identifying details, health and treatment history, what you share in sessions, assessment and test results, correspondence, and billing records. We use them to provide psychological services, to communicate with you about appointments, to bill you or a third party payer, to meet our professional record keeping obligations, and to comply with the law.

Clinical records are kept in Jane, a Canadian electronic records system that complies with PHIPA. Paper records and devices are kept in locked storage at the practice. Access is limited to Heather Hargraves and to the staff and supervisees who need it to do their work, each of whom is bound by confidentiality. Records are retained for at least ten years after your last contact with the practice, in line with the CPBAO record keeping standard, and are then securely destroyed.

The circumstances in which information can be shared without your consent, and the small number of situations in which the law requires us to share it, are set out in full in the consent to services agreement you review and sign before services begin.

3.Assessments requested by a third party

Some of the practice’s work is assessment requested by an insurer, an employer, a lawyer, the Workplace Safety and Insurance Board, Veterans Affairs Canada, or a court, rather than treatment.

In that work the referring party is the client, not the person being assessed. The report goes to the referring party, and the confidentiality rules are different from those that apply in treatment. They are explained to you in writing before the assessment starts.

4.Practitioners working under supervision

The practice includes practitioners working under Heather Hargraves’ supervision. If you see a supervised practitioner, your information is discussed with the supervisor, who is professionally responsible for your care and has access to your record. You will be told this before services begin.

5.Email, text, and virtual sessions

Email, text message, and video carry risks that in person contact does not, including interception and delivery to the wrong recipient.

Virtual sessions are held through Jane Online Appointments. The video connection is encrypted and sessions are not recorded or stored. If you would rather not use electronic communication for any purpose, tell us and we will use another method.

6.Seeing your record, and raising a concern

You have the right to see your record, to ask for a copy, and to ask us to correct information you believe is wrong. Requests should be made in writing, and we respond within the timelines set by PHIPA. There is a limited set of circumstances in which access can be refused, and we will tell you in writing if that applies and why.

7.Using this website

This site provides general information about Hargraves Psychology and the services it offers. Nothing on it is psychological advice, treatment, a diagnosis, or an assessment, and nothing on it should be relied on in place of consulting a qualified professional about your own situation.

Reading this site, sending an email, or submitting a booking request does not create a professional relationship. A professional relationship begins only after an initial appointment, a discussion of the proposed services, and your written consent to services.

Heather Hargraves is registered to practice psychology in Ontario. Psychological services are available only to people who are physically located in Ontario at the time of their appointment.

All content on this site, including text, images, the Hargraves Psychology name and logo, and any frameworks, inventories, or clinical materials described here, is the property of Hargraves Psychology Professional Corporation or is used with permission.
